LET’S TALK

Support organized around everyday function —not just a diagnosis.

Chronic conditions often overlap. The useful question is how they affect this person’s routines, safety, energy, mobility, and ability to live at home.

FUNCTION-FIRST SUPPORT

What nonmedical support can do

Northline begins with functional needs and household reality—not a separate product for every diagnosis.

Northline supports people living with diabetes, chronic obstructive pulmonary disease (COPD), congestive heart failure (CHF), multiple sclerosis (MS), amyotrophic lateral sclerosis (ALS), traumatic brain injury, advanced mobility limitations, and other physical or cognitive conditions.

When several conditions are present at once, the plan has to reflect one person’s actual day—not a checklist of diagnoses.

Several conditions, one daily life

A person may be managing diabetes, heart disease, arthritis, limited mobility, and mild cognitive change at the same time. Northline builds the service plan around the person’s actual day rather than treating each diagnosis as a separate product.

What nonmedical support cannot do

Care Professionals do not assess medical conditions, alter treatment plans, administer services that require a nursing license, or provide emergency response. Clinical tasks must remain with appropriately licensed professionals.

WHEN HOME CARE IS NOT ENOUGH

Clinical nursing and healthcare advocacy

Some situations require licensed nursing or professional healthcare advocacy beyond Northline’s nonmedical scope. Broad Street Advocates & Private Nursing is Northline’s preferred partner for families who need that higher level of support.

Describe the person’s day and where it breaks down.
