Tell us what is happening

The first conversation is an opportunity to describe the person who needs support, what has changed, what the family is currently managing, and what kind of help you believe may be needed. You do not need to have every answer before calling.

Clarify the needs and constraints

A Home Care Administrator will complete a home-care assessment covering daily routines, personal-care needs, mobility, memory, household expectations, location, desired schedule, timing, and other factors that affect service. When clinical questions are present, a licensed nurse may also participate. We will explain what Northline can and cannot provide.

Determine whether Northline is the right fit

Sometimes the request fits traditional home care. Sometimes the person needs skilled nursing, healthcare advocacy, a facility, or another type of support. Honest guidance at the beginning is better than beginning an arrangement that cannot meet the actual need.

Develop a practical service plan

If Northline is appropriate, we establish the requested responsibilities, schedule, communication preferences, and service expectations. We will discuss factors that may affect timing and the availability of Care Professionals.

Prepare to begin care

Northline identifies Care Professionals who can meet the practical requirements of the assignment and prepares them with the information needed to begin. Starting service may happen quickly in some circumstances; other situations require additional time to build responsibly.

Communicate and adjust

Once service begins, your Home Care Administrator remains the primary point of contact. As needs or circumstances change, we work with the family to understand what is different and what adjustments may be available.
